Utilizing a Religious Exemption in the
Commonwealth of Virginia
Religious exemption forms can be
obtained at your local health department or a copy may be downloaded from this
site. Please note that the exemption form does not ask for your religious
affiliation or for a letter from your clergy. This is not necessary and for
anyone to ask this of you is a violation of your constitional rights to
religious freedom. The exemption form must be notorized. Most banks will do this
for free if you have an account with them. Once notarized, keep the original in
a safe place and make copies to give to: *Your child's doctor *Your child's
school or day care and *family members. You may want to keep one in your wallet
in case of an emergency visit to the hospital. It has been suggested that you
staple your attorney's business card to each exemption form.
