“Outbreak of Poliomyelitis—Dominican Republic and Haiti, 2000-2001”

In the Dominican Republic, 12 laboratory-confirmed
poliomyelitis cases attributed to vaccine-derived poliovirus type 1 were identified
between July 2000 and February 2001.
All of the case-patients were either unvaccinated or inadequately vaccinated,
and 11 of the case-patients were six years of age or younger. The date of the onset of paralysis of the
last case was January 2 of this year.
In Haiti, an unvaccinated child aged two years old developed polio
attributed to vaccine-derived type 1 poliovirus, with the onset of paralysis
taking place on August 30, 2000. As of
February 21, three acute flaccid paralysis (AFP) cases from Haiti and 33 AFP
cases from the Dominican Republic were pending final classification. In December 2000 and February 2001, the
Dominican Republic held nationwide mass vaccination campaigns with oral
poliovirus vaccine, and a third round is planned for April 2001. Haiti has also initiated regional oral poliovirus
vaccination campaigns that will be carried out approximately every two
months. [The risk of AFP from vaccine-derived
poliovirus is directly related to a low overall immunization rate in the
population, and is best prevented by a high immunization rate.]
